PAYMENT & INSURANCE
The providers at Summit Health and Wellness have negotiated and contracted with several insurance companies. We will do our best to file with your insurance company in a timely manner. We ask that you update your insurance promptly when changes occur so we have the most recent billing information on file. We will inquire with your insurance company about your insurance benefits, however verification of insurance benefits are not a guarantee of payment.
Please remember that we file insurance as a courtesy to you and any questions or disputes of coverage are between you and your insurance company.
We require that all copays, coinsurances, and deductibles to be paid at the time services are rendered. Services rendered that are not covered by insurance are also due at the time of service.
We are able to file claims with most insurance companies. If we are not contracted with your insurance company you may still have out-of-network benefits that may help cover a portion or your treatment.

Will you cover the programs that Summit Health and Wellness provides?
How much is my deductible?
Do I have a copay, co-insurance?
How many visits per year are covered?
Does my insurance plan require a referral from another doctor, or preauthorization?
